Department Of Health Conducting Community Health Assessment

MAYVILLE – The Chautauqua County Department of Health and Human Services, in conjunction with the Chautauqua County Health Network, Lake Erie Regional Health System, P2 of Western New York, The Chautauqua Center, WCA Hospital and Westfield Memorial Hospital, is conducting a community health assessment to identify and address the most pressing public health needs of Chautauqua County.

County residents are encouraged to participate in this process by taking the Chautauqua County Community Health Survey. The survey can be accessed at www.surveymonkey.com/s/health-chautauqua. A link to the survey is also available on the County Department of Health and Human Services website at myhealthycounty.com.

The survey should take 5 to 7 minutes to complete and will be available through May 31. All individual responses to the survey will be kept confidential, and collective responses will help guide future health programming throughout Chautauqua County.

In addition to this survey, a series of community conversations will be held throughout the months of May and June in the service areas of the county’s four hospitals to gain public input around what obstacles and solutions they see as important to improving health within their community. These events will be held in Dunkirk, Jamestown, Silver Creek and Westfield, and will be hosted by the Chautauqua County Health Network and P2 of Western New York.
